Please bring the following to the GBCHC for your first visit:

1. Your most recent financial information as one of the following

For those who did not file taxes last year or have no income. The IRS can be reached at 1-800-829-1040.

If you cannot provide a 1040 or a 1040ez (tax return), please provide two (2) from the list below:

2. Intake Packet, which includes the following:

3. Letter of Denial from Medicaid

4. A valid picture ID, government-issued

If you do not have all these documents, or have questions, we can help. Call us at 248-309-3752. Clinical medical services provided by the Dr. Gary Burnstein Community Health Clinic are entirely free of charge to our patients.

New Patient Applications will be processed every Tuesday, Wednesday and Thursday from 9am – 3pm. Medical appointments are normally within a few days of receiving all of the paperwork.
